All Adults & Minors participating in on-court activities at Momentous Sports Center must complete a Waiver & Release of Liability.

STEP 3

Required before practice

Important:

  • After completing the waiver, you will receive a QR code via email

  • This QR code is required every time you enter the gym

  • You (or your child) must present this at the Front Desk either print the QR code or have it on your phone ready. This is required for every visit, not just the first time

Once inside the facility at Momentous Sports Center:

  • Look for TV screens around the gym

  • Find “TQ Irvine” listed

  • Your court number will be displayed
